Main duties of the job
As a Planned Care Nurse your responsibilities will vary. You will

Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
Undertake medication round as required, dispense controlled medications and assess patients.
Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
Covering practice nurse duties which entails day to day patient care similar to a ward setting.
Long term condition managment and screening in line with NHS expectations.
Hold and run vaccination clinics.
Inpatient care.
Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
Deliver non-judgemental care that makes a real difference
Inspire excellence by forging strong links with our partners and stakeholders
Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Qualifications required:

Registered nurse Adult
Current professional qualification (NMC)
Evidence of Continued Professional Development
Understanding of the complexities of multi-disciplinary and multi-agency working
A non-judgmental and compassionate approach
Excellent communication skills

Although not essential ideal applicants would have experience in a hospital, community or prison environment.

We have an exciting opportunity for a Planned Care Nurse to join our Planned Care team at HMP/YOI Wormwood Scrubs, a category B Prison located in West London.
Hours
37.5hrs Mon - Fri 7.30am - 4pm includng Bank Holiday
This position is full time, youll receive an annual salary up to £55,086 per annum - depending on experience.

What you will do
