An exciting opportunity has arisen for an experienced, motivated and forward-thinking Band 7 Clinical Nurse Lead to join our Clinical Triage Service at SWFT.
This key clinical leadership role provides clinical oversight, support and guidance to the triage team, ensuring patients receive safe, timely and appropriate care. The service reviews referrals, supports clinical decision-making, prioritises patient need and ensures individuals are directed to the right service at the right time.
As Clinical Nurse Lead, you will provide day-to-day clinical oversight, support staff with complex triage decisions, promote consistent and robust processes, and maintain high standards of care. You will work closely with the Operational Professional Lead and Locality Manager to support the effective running and development of the service.
The role includes contributing to service development, clinical governance, audit, supervision and staff support, ensuring the service remains responsive, safe and patient-focused.
We are looking for an experienced clinician with excellent assessment, communication and decision-making skills who can work confidently in a fast-paced environment. You will be approachable, organised and proactive, lead by example and maintain effective clinical oversight.

Main duties of the job
The post holder is required to be a Registered Nurse with substantial acute/urgent/frailty experience of managing patients with complex needs and holding advanced clinical assessment skills.
To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.
Monitor and lead improvements to standards of care through, supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional and clinical leadership.
The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care.
To work closely with Medical and Nursing staff in both primary and secondary care, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care
The ICC Clinical Nurse Lead is responsible for the clinical triage of the presenting complex health needs of an identified population and for the identification of response required The post holder will be a registered nurse and They will be providing clinical leadership for all patients triaged within the ICC service. They will focus on ways to achieve and demonstrate overall improvements in health care and timely access to services.

Providing highly developed clinical leadership for all patients presenting with an urgent health care need to the clinical triage service through advanced level autonomous practice including taking managed risks, for example where the patient is kept at home, rather than hospital or hospice admittance as would previously been the practice.
Provide highly specialist clinical leadership for patients and work collaboratively with all professionals, carers and relatives to gain a deep understanding of all aspects of the patients physical, emotional and social situation. This includes detailed history taking, red flag identification and identification of appropriate service provision.
Provide highly specialist clinical leadership for the development and maintenance of safe and effective triage of patients based on the full assessment of medical, nursing and care needs.
Ability to understand and consider investigations and tests using clinical guidelines and policies to support clinical decision-making.
As required, maintain competence in prescribing according to level of prescribing qualification i. e. Independent Prescriber and participate in regular continuing professional development in relation to the role.
Liaise with other agencies, including the voluntary sector, to mobilise resources to meet patient needs and to support them at home securing additional support as required.
Act as a resource for the ICC service providing teaching and education to them as required.
To promote new research, innovation and evidence to support quality improvements and evidence-based practice within the team

Provide a visible clinical profile, having both advisory and clinical input into patient care as directed by Head of Community Nursing.
Prepare accurate and concise reports both verbally and written.

Provide care interventions and higher-level assessments for patients with highly complex conditions to ensure that patients achieve their optimum level of wellness.
Autonomous Clinical Practice

Utilise highly specialist triage skills appropriate to Routine Pathway
Analyse signs and symptoms, laboratory tests, investigations and other measures of function in formulating a diagnosis.
Use highly complex analytical and problem-solving skills in unpredictable situations.
Provide evidence-based treatment plans based on sound clinical decision making, including the unique presentation of illness and response to treatment of patients on the community nursing caseload.
Collaborate with the multi-disciplinary team to plan and implement treatment for acute illness/injury/chronic illness as required.
Negotiate, collaborate and support individuals and families to make sometimes difficult decisions about plans of care by empowering them to decide on the most appropriate course of action.
Ensure a robust process is in place to review care and treatment plans.
Take a leading role in auditing and monitoring all components of care delivery.
Review and develop clinical competency as required to support clinical practice, participating in ongoing personal development of self and of others.
Ensure all clinical records are kept in line with SWUF and NMC guidelines.
Communication
Establish the dynamic process of understanding, reflecting, active listening and checking understanding, thus developing a therapeutic relationship with patients and their carers.
Enable and support patients as active partners in the care planning process for present and future needs.
Dissemination of information throughout the multidisciplinary team, taking into account the patients right of confidentiality
Education
Utilise communication skills and knowledge to promote patients and families abilities to make informed decisions and participate in care planning.
Participate in the development of the teams knowledge for Urgent Care Pathways
Work alongside team managers to support staff in clinical triage and assessments unfamiliar to them.
Continue to develop own expert practice.
Critically evaluate research and educate the team to ensure that this theory is integrated into practice.
Role model and support staff to ensure that they are clinically competent to undertake new complex skills which are maintained to ensure that patients receive the highest possible standards of care in the most appropriate environment as close to home as possible, preferably in their own home if that is their express wish.

Essential criteria
- Professional/clinical knowledge acquired through degree, supplemented by specialist training, experience, short courses, CPD to master's degree level or equivalent
- Registered Nurse
- PGDip District Nursing or relevant equivalent qualification e. g. Clinical Practitioner, Frailty
- Additional Leadership CPD - e. g. Mary Seacole Programme, LEO Programme or equivalent qualification.
- Experience/relevant qualification in mentoring or skills/experience in Coaching
- Participated in Quality improvement initiatives
- Significant post registration experience as a senior nurse in an acute/urgent/frailty setting
- Experience of leadership and leading change or project implementation
- Experience of undertaking comprehensive health needs assessment and developing complex care plans
